Abstract:
A customizable and dynamic electronic patient form and information system is provided having features whereby a healthcare provider may select and record various values corresponding to patient categories, the values being recorded as discrete data points at locations on a display that are reliable and predictive, and that are simultaneously displayed as selections on the patient form and as entries on a data logger or patient summary sheet. Further changes or modifications to the value are recorded as discrete data points and simultaneously displayed on the patient form and in the patient summary sheet. The patient form further includes features to permit a user to modify the form as desired to suit the needs of the patient and/or the healthcare provider's field of practice.

Abstract:
A record and billing service system and method for use in health care offices. The system allows a health care agent to generate a visit form for a patient that keeps a record of the patient's health history and treatment records received at a healthcare facility. The visit form includes procedure and/or diagnoses information that is particular to a given health care provider or facility. Thus, the system generates the visit form, which shows the types of procedures, diagnosis, inventory, and the like normally provided in the health care office by the health care provider. The application can be customized to reflect specialties provided in that office or by that provider. The system also converts the types of procedures, diagnosis and inventory matters listed in the application into accurate billing records.

Abstract:
A multi-channel switchmode audio amplifier is configured by a programmed processor such that each channel drives separate loads, is connected in parallel or is configured in a bridge-tied mode as well as combinations thereof. In one embodiment, amplifier channels that are connected in parallel have power amplifiers that are driven with signals from a single modulator. A feedback circuit and error amplifier from one channel controls the modulated signal that is applied to each parallely connected amplifier channel. Current feedback circuits for parallely connected amplifier channels are eliminated by tightly controlling the timing of switching in the power amplifier output stages.
